This position provides support coordination to individuals approved under the Developmental Disabilities (DD) Waiver program. The role involves working closely with consumers, families, and community supports to ensure that assigned consumers receive services at the least restrictive level of care.

Essential Duties and Responsibilities:

  • Provides case management services for individuals approved under the Developmental Disabilities Waiver.
  • Conducts in-person assessments and planning services with individuals to determine needs for medical, behavioral health, educational, social, or other services/supports.
  • Develops individual support plans (ISP) in compliance with Home and Community Based Settings (HCBS) regulations.
  • Links individuals to necessary medical, social, educational providers, or other supports and services, ensuring alignment with ISP outcomes.
  • Assesses ongoing progress and ensures delivery of outlined services, addressing any status changes.
  • Coordinates services with other agencies and providers on the care team.
  • Maintains client confidentiality and charts, ensuring compliance with program standards and regulatory requirements.
  • Prepares case file documentation, adhering to regulations relevant to the Developmental Disabilities Waiver.
  • Records documentation in electronic health record systems and serves as a point of contact for external agencies on behalf of clients.
  • Participates in agency committees, collects program data, and performs routine day/evening travel for client visits and other duties.
  • Represents the Agency professionally in the community and workplace, demonstrating cultural sensitivity.

Qualifications:

  • Bachelor’s Degree in a human services field required.
  • Developmental disability work experience or relevant education to meet knowledge, skills, and ability requirements.
